Abstract

Ships require regular maintenance and repair through dry-docking activities. One of the facilities used for this purpose is a floating dock that floats above the sea. Its operation is susceptible to failure because it depends on structural stability, component performance, and operator accuracy. At a shipyard in Surabaya, 70 cases of component failure were recorded during 2024–2025, resulting in operational disruptions and project delays. This study aims to identify the most critical failure mode, determine its root causes and failure probability, and propose appropriate risk control measures. The methods employed were Failure Mode, Effects, and Criticality Analysis (FMECA) and Fault Tree Analysis (FTA). Data were obtained from failure records, field observations, and expert judgment. The FMECA results identified the distributing valve as the most critical component. Failure mode DV-24.2, in which the valve failed to open or close, had the highest criticality ranking of 54.08, with a severity value of 8, a criticality number of 6.76, and 13 recorded failures. The FTA identified 10 minimal cut sets, comprising seven first-order and three second-order, with a top-event probability of 1.0000. The proposed risk controls include replacing damaged components, strengthening automatic and manual control systems, verifying draft calculations, improving maintenance activities, and using appropriate personal protective equipment.
