ABSTRAK PT Petrokimia Gresik merupakan salah satu perusahaan yang memproduksi berbagai jenis pupuk dan bahan kimia. Perusahaan ini telah menerapkan golden safety rules, yaitu terdiri dari 5 peraturan utama yang harus dipatuhi oleh seluruh karyawan di PT Petrokimia Gresik, dimana peraturan tentang keselamatan lalu lintas adalah salah satu peraturan yang di terapkan. Tetapi berdasarkan data kecelakaan lalu lintas di PT Petrokimia Gresik pada tahun 2013 hingga 2015 angka kecelakaan terus mengalami peningkatan, total terdapat 32 kasus kecelakaan lalu lintas. Dan tidak sedikit kasus kecelakaan lalu lintas di perusahaan ini disebabkan oleh faktor lingkungan jalan. Berdasarkan pengamatan peneliti saat kerja praktek selama 4 bulan di PT PKG kondisi jalan di perusahaan tersebut banyak mengalami kerusakan yang sangat mengganggu bagi pengguna jalan. Karena itu diperlukan suatu penelitian yang bertujuan untuk mengidentifikasi faktor-faktor yang berpengaruh terhadap kecelakaan lalu lintas, melakukan penilaian terhadap kondisi jalan untuk mengetahui tidakan pemeliharaan/perbaikan yang sesuai. Penelitian ini menggunakan metode pavement codition index (PCI) untuk mengetahui jenis dan tingkat kerusakan jalan yang di gunakan sebagai acuan dalam usaha pemeliharaan, menggunakan uji korelasi pearson product moment dan regresi linier sederhana dengan taraf signifikan ( = 0,05) untuk mengetahui pengaruh antara variabel terikat (jumlah kecelakaan lalu lintas) terhadap variabel tidak terikat (faktor penyebab, waktu kejadian, jenis kendaraan dan jenis pekerjaan) Berdasarkan hasil uji korelasi menggunakan program SPSS 20.0 diperoleh bahwa faktor-faktor yang berhubungan terhadap jumlah kecelakaan lalu lintas adalah faktor penyebab yaitu, faktor kerusakan jalan, faktor manusia dan faktor kendaraan, faktor waktu kejadian yaitu, pukul 06.00 s/d 12.00, pukul 12.00 s/d 18.00 dan 18.00 s/d 24.00, faktor jenis kendaraan yaitu kendaraan roda dua dan empat, faktor jenis pekerjaan yaitu karyawan pabrik 2, 3 dan non organik. Untuk pengaruh faktor kerusakan jalan terhadap kecelakaan lalu lintas di dapatkan persamaan regresi sebesar Y = 0,358 + 1,006 X. Hasil metode PCI diperoleh 11 jenis kerusakan jalan dan nilai index kondisi perkerasan jalan di PT Petrokimia Gresik adalah 54,3. Maka kondisi ruas jalan PT Petrokimia – jalan Tridharma termasuk dalam kategori buruk. Kata kunci : Kecelakaan lalu lintas, pavement condition index (PCI), korelasi, regresi ABSTRACT PT Petrokimia Gresik is one of the company that produces various types of fertilizers and chemicals. The company has implemented a golden safety rules, that consists 5 main rules that must be followed by all employees of PT Petrokimia Gresik, where traffic safety regulations is one of the applied regulation. But based on PT Petrokimia Gresik traffic accidents record in period 2013 until 2015, the number of accidents continually increase, there were 32 traffic accidents cases occurre. And many traffic accidents cases in the company are caused by road condition factor. Basde on researchers observations on the job training for 4 months that the road condition was not worthy that distrub the road users. Therefore we need research to identify the factors that influence on traffic accidents, perform assessment of road conditions to know the repair/maintenance action accordingly. This research used pavement codition index (PCI) method to find out the type and level of the damage to roads in used as a reference in the business of maintenance, using correlation pearson product moment test and a simple linear regression with significant levels ( = 0.05) to know the influence between the dependent variable (the number of traffic accidents) against the independent variable (cause factor, time of occurrence, type of vehicle and the type of profession) Based on the results of correlation test is using SPSS 20.0 software obtained that factors related to the number of traffic accidents is a causative factor is, factors of road damage, the human factor and the factor of the vehicle, the time factor is events, 06.00-12.00, 12.00-18:00 and 18:00-24.00, factor in the type of vehicles are two-wheelers and four, factor in the type of work that employees of the factory 2, 3 and non-organic. To the influence of road damage to the traffic accident get the regression equation is Y = 0,358 + 1,006 X. PCI method results 11 types of damage to the road and the road roughness conditions index value in PT Petrokimia Gresik is 54.3.
