ABSTRAK Salah satu cara pencegahan kecelakaan kerja adalah pemakaian Alat Pelindung Diri (APD) dengan tepat. PT. Cheil Jedang Indonesia berkomitmen untuk menciptakan kecelakaan nihil dengan cara menyediakan alat pelindung diri untuk karyawannya di tempat kerja. Masalah yang sering terjadi adalah kehabisan stok APD pada waktu yang sangat diperlukan dikarenakan pengambilan dan permintaan APD oleh karyawan tidak sesuai dengan yang dibutuhkan. Dengan kondisi tersebut maka diperlukan system informasi manajemen alat pelindung diri yang baik agar pengelolaan dapat dilakukan secara baik dan benar tujuannya untuk mengurangi kecurangan yang sering terjadi di PT. Cheil Jedang Indonesia. Sistem informasi manajemen alat pelindung diri ini dibuat menggunakan metode media berbasis web. Informasi dibentuk melalui web agar seluruh pekerja dapat mengetahui informasi alat pelindung diri yang dipakai telah memasuki batas waktu pergantian berdasarkan perhitungan mean time to failure (MTTF) dari data permintaan dan penukaran alat pelindung diri karyawan. Pembuatan sistem informasi manajemen dibuat menggunakan PHP Hypertext Preprocessor sebagai bahasa pemrograman dan juga menggunakan My SQL sebagai database server. Awal pembuatan sistem informasi manajemen adalah menyusun database dan merancang tampilan web. Kemudian disusun dan di desain menjadi sistem infomasi managemen yang siap digunakan. System informasi permintaan dan pelaporan alat pelindung diri telah diuji coba sesuai fungsi yang diperintahkan. Dari hasil pembuatan dan pengujian web program ini diketahui bahwa data yang sudah diinputkan dapat menunjukkan data rekap laporan permintaan serta karyawan dapat melihat sejarah permintaan. Selain itu form laporan dapat dibuat, diujikan, dan dicetak yang kemudian dapat dievaluasi di PT. Cheil Jedang Indonesia. PT. Cheil Jedang Indonesia was committed to create zero accident by providing Personal Protective Equipment for their worker in work place. Based on that reason, the company needs management information system to management of personal protective equipment can decrease the dishonest upon distribution of personal protective equipment that often happen on PT. Cheil Jedang Indonesia. Management information system of personal protective equipment could be built by media web-base. Information will be collected by web so all worker could get the information whether their personal protective equipment (PPE) has reached the time for exchange the newest one based on meant time to failure (MTTF) of exchanging PPE data’s or not. Management information system was built by PHP Hypertext Preprocessor as the programming language and also MySQL as database server. The beginning of management information system was arranging database and creating the web interface, then it all combine and design to become a management information system that could be ready to use. Management information system request and report of personal protective equipment has been tested according to the function that was ordered. The results from the creating and testing this web can show the summarized data report requests and the employees can see their history PPE requests. In addition, the form report can be printed then be evaluated in PT. Cheil Jedang Indonesia by HSE employee.
