ABSTRAK PT Pertamina Geothermal Energy Area Ulubelu merupakan pengusahaan pengembangan energy panans bumi di Lampung. Dalam proses produksinya uap mulai dari hulu hingga ke hilir tentu saja terdapat potensi bahaya yang cukup tinggi khususnya pada kegiatan proses operasi dan produksi, banyak terjadi kegagalan proses didalamnya diantaranya high level di separator, high pressure di separator dan lain-lain. Adanya potensi bahaya tersebut tentunya dapat dianalisa sehingga tidak mengganggu jalanya proses operasi dan produksi yang akan merugikan keselamatan pekerja dan lingkungan. Untuk menganalisa high risk sebelumnya dilakukan identifikasi bahaya dan analisa risiko menggunakan metode Hazard Operability Study (HAZOP) dengan dilakukanya risk assessment pada setiap identifikasi, dari hasil risk assessment akan didapatkan risk score, yang selanjutnya hasil dari risk score paling tinggi maka dapat dikategorikan sebagai high risk. Kemudian dari risk score tertinggi dianalisa menggunakan metode Event Tree Analysis (ETA) dan Fault tree Analysis (FTA). Untuk mengembangkan rangkaian kecelakaan hasil dari ETA dan FTA, selanjutnya dilakukan Cause Consequence Analysis (CCA), sehingga didapatkan accident sequence minimal cut set. Dari hasil identifikasi yang telah dilakukan didapatkan 3 kategori high risk didapatkan 3 kategori high risk dengan nilai risiko sebesar 16. Dari ke 3 kategori high risk diatas diantaranya PCV-008 stuck menutup terdapat di node 5 pada parameter pressure, LV-001 gagal membuka pada node 5 pada parameter level, dan Meningkatnya tekanan di sumur injeksi untuk node 7 pada parameter pressure. Dan dari CCA didapatkan hasil pengembangan antara initiating event dan basic cause yang dijelaskan pada accident sequence minimal cut set yang memyebabkan terjadinya suatu kecelakaan types of event adalah environment, life time equipment, active equipment failure, dan human error. Kata Kunci : CCA, Event Tree Analysis, Fault Tree Analysis, HAZOP ABSTRACT PT Pertamina Geothermal Energy Ulubelu Area is a company that does business in geothermal energy utilizations. Naturally in its production process from upstream to downstream there will be high potential hazards especially in the process of operation and production activities, such as high level and high pressure in separator, among many others. Those potential hazards can certainly be analyzed so as not to disturb the process of operation and production activities which would be detrimental to the safety of workers and environment. Prior analyzing the high risk, hazard identification and risk analysis will be carried out beforehand using Hazard and Operability Study (HAZOP) method with the execution of risk assessment on each identification. From the results of the risk assessment, risk score will be obtained, then the highest risk score can be categorized as high risk. After that, the highest risk score will be analyzed using Event Tree Analysis (ETA) and Fault Tree Analysis (FTA) method. To develop a series of accidents result from ETA and FTA, Cause Consequence Analysis (CCA) then performed, thus obtaining an accident sequence minimal cut set. The identification that was carried out before resulted in 3 high risk category with the risk score of 16. Those 3 are, PV-008 stucked shut in node 5 on the pressure parameter, LV-001 failed to open at node 5 on the level parameter, and increased pressure in the injection well for node 7 on the pressure parameter. From CCA, it is known that development result between initiating event and basic cause described in accident sequence minimal cut set that causing types of event accident are environment, life time equipment, active equipment failure, and human error.
