Abstract

ABSTRAK PT. X merupakan perusahaan sub contractor yang bergerak di bidang maintenance baik dalam hal konstruksi, mekanik, listrik dan lain-lain. Berdasarkan data kecelakaan PT. X tahun 2011-2013, kecelakaan yang disebebkan oleh Human Error sering terjadi pada saat kegiatan maintenance. Untuk mencegah timbulnya kecelakaan kerja, diperlukan analisis human error dalam tiap tahapan pekerjaan pada ketinggian. Analisis human error sangatlah dibutuhkan karena dapat mengidentifikasi langkah kerja yang dilakukan oleh manusia dan mampu memperbaiki error berdasarkan instruksi kerja. Penelitian ini menggunakan Standardized Plant Analisys Risk Human Reliability Assessment (SPAR-H) untuk menganalisa Human Error Probability (HEP) berdasarkan work instruction masing-masing pekerjaan ketinggian. Populasi yang dianalisis adalah pekerja shift pagi 10 orang pekerja ketinggian pada pekerjaan pemasangan dinding asbes dan penggantian ducting saat shutdown berlangsung selama sebulan. Hasil penelitian menunjukkan bahwa HEP tertinggi untuk pekerjaan ketinggian pemasangan dinding asbes yaitu membongkar dinding asbes dan memasang dinding asbes untuk HEP action sebesar (0.20) dan HEP diagnosis sebesar (0.716). Untuk pekerjaan ketinggian pemasangan ducting yaitu pada pekerjaan mengganti ducting yang baru dan menyambung ducting dengan cara mengelas untuk HEP action sebesar (0.20) dan HEP diagnosis sebesar (0.716). Pengurangan human error dapat dilakukan dengan rekomendasi: melakukan toolbox meeting, pemasangan papan informasi pekerjaan, training kepada semua pekerja ketinggian, penggunaan kartu STOP, perbaikan prosedur kerja, pemberian kotak P3k di setiap unit dan pemberian tempat ex material. X in 2011-2013, accidents caused by human error often occurs during maintenance activities. To prevent occupational accidents, human error analysis is required in each phase of work at height. Analysis of human error is necessary because it can identify the steps work done by humans and able to fix the error based work instructions. This study uses the Standardized Plant Risk Analisys Human Reliability Assessment (SPAR-H) to analyze Human Error Probability (HEP) based on the work instruction of each job altitude. The population analyzed is the morning shift workers 10 workers at the height of the installation work and the replacement of asbestos wall ducting when the shutdown lasts for a month. The results showed that the highest HEP for the work altitude wall mounting asbestos is asbestos dismantle the wall and put up a wall of asbestos for HEP action amounted to (0.20) and HEP diagnosis of (0716). To work ducting installation height is at work replacing the new ducting and connecting ducting by means of welding to HEP action amounted to (0.20) and HEP diagnosis of (0716). Reduction of human error can be done with the recommendation: do toolbox meetings, installation of information boards work, training to all workers altitude, STOP card usage, improved work procedures, giving P3K boxes in each unit and the provision of a material ex.
