Abstract

ABSTRAK Berdasarkan rekapan data kecelakaan kerja PT. X tahun 2011-2015 diketahui bahwa kecelakaan kerja pada proses pekerjaan welding dan pekerjaan ketinggian menempati urutan teratas yang disebabkan oleh human error. Untuk mencegah timbulnya kecelakaan kerja diperlukan analisa human error dalam tiap tahapan tugas pekerjaan welding dan pekerjaan pada ketinggian. Analisa human error sangatlah dibutuhkan karena bertujuan untuk mengidentifikasi langkah kerja yang dilakukan oleh manusia dan mampu memperbaiki error yang sebenarnya berdasarkan instruksi kerja. Penelitian ini menggunakan salah satu metode dari Human Reliability Analysis (HRA) yaitu Cognitive Reliability and Error Analysis Method (CREAM ) untuk mendapatkan nilai Cognitive Failure Probability (CFP). Langkah yang dilakukan ada dua cara adalah yaitu basic method dan extended method. Pada tahapan basic method didapatkan nilai CFP secara umum yang didapat dari penilaian Commond Performance Condition oleh expert judgement . Pada tahapan extended method akan didapatkan nilai CFP untuk setiap task. Berdasarkan hasil penelitian menunjukkan faktor-faktor yang mempengaruhi timbulnya error pada pekerjaan welding dan pekerjaan pemasangan dinding asbes dan pekerjaan penggantian ducting adalah kecukupan organisasi, kecukupan Man Machine Interface (MMI), jumlah tugas yang dilakukan, kecukupan pelatihan dan pengalaman, ketersediaan prosedur. Aspek kognitif yang memiliki nilai probabilitas error paling tinggi pada pekerjaan welding adalah execution dengan nilai CFP sebesar 0.18, selanjutnya aspek kognitif yag memiliki nilai probabilitas error paling tinggi pada pekerjaan pemasangan dinding asbes adalah Execution dengan nili CFP sebesar 0.0576 dan aspek kognitif yang memiliki nilai probabilitas error paling tinggi pada pekerjaan penggantian ducting adalah planning dengan nilai CFP sebesar 0.03. Sebagai rekomendasi work instruction sebaiknya dibuat lebih rinci, memberikan sosialisasi penggunaan alat dan memberikan training untuk bidang welding dan bidang ketinggian. X years of 2011-2015, it is known that the highest occupational accident happen during the welding process and heights occupation. The data also shows that most of the accidents caused by human error. In order to prevent the occupational accidents, it requires an analysis of human error in every stage of welding process and work at height. Analysis of human error is much needed as it aims to identify the working stages done by workers and fixing the actual error based on the work instructions. This study use one of the methods from Human Reliability Analysis (HRA) which is Cognitive Reliability and Error Analysis Method (CREAM). This method used to get the value of Cognitive Failure Probability (CFP). There are two steps that used in this method, which are basic method and extended method. For the basic method, general CFP value obtained from Commond Performance Condition assessment by expert judgment. While for the extended method, CFP values obtained will be obtained from each task. Based on the results, it shows that there are several factors that trigger the incidence in welding and the work of asbestos walls and ducting replacement installation. Those factors are the adequacy of the organization, adequacy of Man Machine Interface (MMI), the number of tasks performed, the adequacy of training and experience, and the availability of the procedure. Cognitive aspect which has the highest probability of error is welding and execution with CFP scores 0.18. Then, the cognitive aspect that has the highest probability of error in asbestos installation is execution with CFP scores 0.0576. The last cognitive aspect that has the highest probability of error in replacing ducting is planning with CFP scores 0.03. Therefore, as the recommendation from this research, work instruction needs to be made with more detail explanations, providing socialization tools and provide training for the field of welding and height work.
