ABSTRAK PT Petrokimia Gresik merupakan industri yang bergerak di bidang industri kimia dan pupuk dengan produk utamanya berupa Pupuk Nitrogen dan Pupuk Fosfat. Selain itu PT Petrokimia Gresik juga memproduksi bahan bakunya, antara lain amoniak, asam sulfat, dan asam fosfat. Amoniak merupakan bahan kimia berbahaya dan beracun (B3). Risiko kebocoran amoniak akan mengakibatkan bahaya kesehatan cukup tinggi jika terlepas ke lingkungan pabrik dan pemukiman warga. Oleh karena itu diperlukan suatu kajian untuk mengidentifikasi dan menganalisis risiko kebocoran serta tingkat kesiapsiagaan PT Petrokimia Gresik, sehingga dapat dilakukan tindakan pengendalian atau penanggulangan bencana. Penelitian ini akan mengkaji tentang analisis risiko pada ammonia storage tank (32-TK0801) menggunakan HAZOPS. Selanjutnya dilakukan perhitungan konsekuensi menggunakan dispersion models untuk mengetahui laju pelepasan amoniak, konsentrasi, dan jarak paparan. Perhitungan akan berdasarkan pada dua kasus yang mungkin terjadi yaitu lepasnya amoniak cair pada dasar tangki melalui pipa karena terjadinya kegagalan sistem dan lepasnya uap amoniak pada atap tangki karena kerusakan tangki. Setelah itu dilakukan penilaian kesiapsiagaan menggunakan kuesioner CAR untuk mengevaluasi manajemen bencana dan menyusun langkah mitigasi berdasarkan hasil identifikasi bahaya dan kuesioner CAR. Dari hasil penelitian didapatkan bahwa nilai risiko pada ammonia storage tank mencapai tingkat risiko ekstrim. Radius aman (25 Ppm) untuk kebocoran pada dasar tangki adalah 199,100599 Km dan 221,6445165 Km untuk kebocoran pada atap tangki. Berdasarkan readiness assessment menggunakan kuesioner CAR diketahui bahwa penilaian tingkat kesiapsiagaan PT Petrokimia Gresik terhadap kebocoran ammonia storage tank termasuk kedalam area meeting criteria dengan nilai 2,3901. Tindakan mitigasi struktural berupa perbaikan secara fisik. Sedangkan mitigasi nonstruktural berupa upaya perbaikan sistem manajemen. peraturan atau prosedur yang berlaku berdasarkan hasil kuesioner CAR. Kata kunci : ammonia storage tank, HAZOPS, dispersion models, CAR, mitigasi. ABSTRACT PT Petrokimia Gresik is a company that produces in chemical and fertilizer industry. Their main product are nitrogen fertilizer and phosphate fertilizer. They are also independently producing their own raw material such as ammonia, sulfuric acid, and phosphoric acid. Ammonia is a chemical hazard. Leakage risk of ammonia will impact to dangerous health effect of people around the plant. That‟s why we need a research to identify and analyze the risk and the level of preparedness of PT Petrokimia Gresik, so we can prevent and control the disaster. This research used to analyze the risk of ammonia storage tank‟s leakage (32- TK-801) using HAZOPS. Dispersion models used to determine the release rate of ammonia, concentration, and distance. The Consequence could be obtained from two dispersion model, which were ammonia liquid release throught the pipe on the ground level due to system failure and ammonia vapour release above ground level cause of the tank was damaged. After that, doing readiness assessment using CAR questionnaires to evaluate their disaster management evaluate and compose mitigation based on the results of hazard identification and CAR questionnaire. In this research showed that the risk of ammonia storage tank reaches extreme levels. Safe radius (25 ppm) for leak at the ground level is 199.100599 Km and 221.6445165 Km for leak above ground level. Based on the readiness assessment using CAR questionnaires, researcher identify that the emergency response of PT Petrokimia Gresik was include in area meeting criteria and the score were 2,3901. The structural mitigation is physical improvement and the non-structural mitigation is management system improvement, regulations or procedures based on the results of CAR questionnaires.
