ABSTRAK PT. Petrokimia Kayaku merupakan sebuah perusahaan yang bergerak dibidang formulasi pestisida dan distribusi bahan kimia pertanian. Pada PT Petrokimia Kayaku tahun 2013 terdapat 37 kasus telah terdapat kasus kecelakaan kerja seperti keracunan, terpercik cairan bahan jadi dari mesin filling, tangan melepuh, tangan dan kaki terjepit dan terkena semburan api pada pekerja produksinya. Pada tahun 2014 terjadi 26 kasus. maka dari itu untuk meminimalisir kecelakaan kerja dilakukan penelitian analisis kecelakaan kerja dengan menggunakan metode Why Because Analysis dan Technique of Operating Review. Why-Because Analysis ( WBA ) merupakan suatu metode yang memberikan kerangka analisa secara menyeluh dan lengkap terhadap kegagalan sistem dan penyimpulan tingkat keselamatan sistem.Technique of Operating Review ( TOR ) pada awalnya dikenalkan oleh Weaver pada tahun 1973 sebagai alat pencegahan kecelakaan dan pelatihan diagnostik. TOR juga dapat digunakan sebagai teknik investigasi kecelakaan. Fokus analisa TOR adalah pada kegagalan sistem, dan pencarian untuk mengidentifikasi kegagalan manajemen. Setelah dilakukan identifikasi dan analisa terhadap kasus kecelakaan di PT. Petrokimia Kayaku dengan metode Why Because Analysis dan TOR maka dapat disimpulkan bahwa penyebab dasar terjadinya kecelakaan tersebut adalah Dari pekerja tidak memakai masker maupun APD yang lain pada saat bekerja dan menurunnya fokus pekerja terhadap mesin dan pekerjaan, Kondisi area kerja tidak bersih dikarenakan masih terdapat ceceran bahan produk pada lantai kerja yang dapat menimbulkan kecelakaan Tidak adanya karet pengaman pada tepi pintu mesin dan setelan pintu menutup secara otomatis Tidak terdapat pelindung pada forklift yang membuat pekerja terlindungi ketika diharuskan dekat dengan forklift.Rekomendasi yang dapat diberikan kepada perusahaan untuk meminimalisir tingkat kecelakaan terhadap pekerja antara lain Pembuatan check point APD bagi para pekerja dan staf yang akan memasuki area terbatas produksi, jika tidak lengkap APD yang digunakan otomatis pekerja maupun staf tidak dapat memasuki area terbatas produksi Pemberian karet pengaman pada tepi pintu mesin agar ketika menutup akan melindungi pekerja apabila bagian tubuh pekerja masih di dalam mesin Pemberian sirine dan rotary light yang tersambung dengan mesin forklift, apabila mesin forklift menyala sirine akan berbunyi “beep“ dan rotary light akan menyala sehingga pekerja lain akan mengetahui bahwa forklift tersebut sedang beroperasi. Petrokimia Kayaku is a company engaged in the formulation of pesticides and agricultural chemicals distribution. At PT Petrochemical Kayaku in 2013 there were 37 cases there have been cases of occupational accidents such as poisoning, liquid splashed finished material from the filling machine, blistered hands, arms and legs pinned and exposed to flame on its production workers. In 2014 happened 26 cases. therefore to minimize work accidents crash analysis research work using Why Because Analysis and Technique of the Operating Review. Why-Because Analysis (WBA) is a method that provides a framework menyeluh and complete analysis of the failure of safety systems and inference levels sistem.Technique of Operating Review (TOR) was originally introduced by Weaver in 1973 as a means pencegahankecelakaan and diagnostic training. TOR also can be used as an accident investigation techniques. TOR analysis focus is on system failure, and the quest to identify the failure of management. After the identification and analysis of accident cases in PT. Petrochemical Kayaku with Rev method Analysis and TOR Because it can be concluded that the basic cause of the accident was of the workers did not wear masks and other PPE at work and decline in workers focus on the machine and work, conditions of work area clean because there are no material spills work product on the floor that can lead to accidents absence of safety rubber on the edge of the machine door and the door closes automatically setting There is no protection on the forklift which makes the workers are protected when it is required to close the forklift.Rekomendasi that can be given to companies to minimize the accident rate for workers between Making another check point PPE for workers and staff who will be entering a restricted area of production, if not complete PPE used auto workers and staff could not enter the restricted area Giving rubber production safety at the edges of the door so that when the machine will shut down to protect the worker if the worker's body parts Provision is still in the machine and rotary light siren connected to the engine forklifts, forklift when the machine turns on the siren will sound "beep" and the rotary light will light up so that other workers would know that the forklift is operating.
