ABSTRAK PT. Z telah berupaya dalam penerapan K3 agar tidak terjadi kecelakaan, pada kenyataanya accident masih terjadi pada bulan Januari hingga Oktober tahun 2014. Berdasarkan data kecelakaan kerja tahun 2014 telah terjadi 10 accident pada PT. Z (sumber: data kecelakaan perusahaan tahun 2014). Adapun kriteria accident tersebut adalah 4 medical treatment, 4 first aid, dan 1 loss time injury. Menghadapi permasalahan tersebut, penulis ingin membuat suatu sistem informasi mengenai kerja aman yang difokuskan dalam penerapan BBS di PT. Z. Seiring dengan perkembangan teknologi, sistem informasi tersebut dapat dibuat dalam basis web. Dengan penerapan sistem informasi manajemen akan mempermud ah penerapan program Behaviour Based Safety sebagai bahan evaluasi perusahaan sehingga perusahaan dapat melakukan perbaikan safety culture dan nantinya perusahaan memiliki database terkait sistem kerja aman yang sistematis sehingga dapat diakses dengan cep at dan aman. Pembuatan Sistem Informasi Manajemen Sistem Kerja Aman ini menggunakan bahasa pemrograman PHP dengan database server MySQL. MySQL adalah suatu sistem manajemen database. Database adalah suatu koleksi data terstruktur. Data tersebut dapat berupa apa saja, dari list sederhana sampai sebuah galeri gambar. PHP atau Pre Hypertext Processor adalah bahasa pemrograman yang berjalan di dalam server, dan mampu membuat web menjadi interaktif dan dinamis. PHP dapat mengolah data dari komputer client dan dari komputer server itu sendiri, sehingga mudah disajikan dalam browser. Dengan sistem informasi menggunakan PHP dan MySQL ini diyakini akan menunjang perbaikan safety behaviour. Perancangan Sistem Informasi ini pada akhirnya menghasilkan output berupa rekapan data kecelakaan dan persentase safe pada sebuah perusahaan yang disajikan dalam sebuah pie diagram dan sebuah diagram batang. Sistem informasi ini juga memberikan output daftar-daftar HIRA, JSA, Work Permit, Safe Card yang telah dilaporkan atau dibuat dan dapat diunduh berupa data.pdf. Sebagai indikasi bahwa perancangan Sistem Informasi berhasil membuat sebuah Sistem Informasi yang sistematik dan mudah untuk digunakan, maka dilakukan survey dalam bentuk kuesioner yang diberikan kepada 10 responden acak. Dari 10 responden, semuanya menyatakan bahwa Sistem Informasi ini sistematik dan mudah untuk digunakan. Kata Kunci : Behaviour Based Safety, HIRA , JSA, Safe Card, Sistem Informasi Manajemen, Work Permit. ABSTRACT PT. Z has commited in appliance of occupational health and safety to prevent accidental, in fact accidents still occurred since January to October in 2014. Depend on company accidents data on 2014, 10 accidents occurred at PT. Z (source: company accidents data 2014). There are some accidents criteria that already happened. The accidents were 4 medical treatments, 4 first aids, and 1 lost time injury. To face the problem, writer want to make an Information Management System in purpose to maintain safe works that focused to BBS appliance at PT. Z. While technology developed fastly, Information Management System can be built as web by localhost system. Information Management System hopefully will make easy the use of Behaviour Based Safety program to be as a part of company evaluations data. Then, the evaluation data will be developed to make a better safety culture. The compay will have databases related to safe work system which systematic and accessed safely and comfortable. To build an information management system of safe work system, the writer use PHP and MySQL language. MySQL is a database management system. Database is a structured data library. The data can be anything, from simple lists until a picture gallery. PHP or Pre Hypertext Processor is a language which run in a server and able to change web into interactive and dynamic web. PHP can develop data from client computer and server computer and make it easy to display in a browser. With PHP and MySQL system information, the writer believes it can improve safety behaviour. Finally, the informatiom system construction give some outputs, the outputs are accident summarize data and safe percentage of a company that served to the viewers as a pie and a bar chart. The other outputs of this information system are lists of HIRA, JSA, Work Permit, and Safe Card that reported or made and can be downloaded as .pdf file. As indications that the writer has built a systematic, easy, and comfortable information system, he did a survey with quesionairs which given to 10 respondens. The final result from the quesionair, all respondens stated that this information system is easy and comfortable to use and systematic.
