ABSTRAK PT. Pelindo III Cabang Tanjung Perak merupakan perusahaan dalam lingkup BUMN di Indonesia. Perusahaan ini merupakan pelabuhan terbesar ke-2 di Indonesia dan menjadi pusat produksi dan distribusi barang domestik maupun internasional. Dikarenakan menjadi pusat produksi dan distribusi terbesar ke-2 dan memiliki potensi bahaya tinggi, maka PT. Pelindo III cabang Tanjung Perak telah berkomitmen dalam sistem penerapan K3 (Keselamatan dan Kesehatan Kerja) pada wilayah kerjanya. Adanya bukti penerapan K3 antara lain dengan pengadaan Alat Pelindung Diri (APD). Sistem informasi manajemen alat pelindung diri dapat diciptakan dengan media berbasis web. Informasi dibentuk melalui web agar seluruh pekerja dapat mengetahui informasi alat pelindung diri yang dipakai telah memasuki batas waktu pergantian berdasarkan perhitungan mean time to failure (MTTF) dari data penukaran alat pelindung diri karyawan. Pembuatan sistem informasi manajemen dibuat menggunakan PHP Hypertext Preprocessor sebagai bahasa pemrograman dan juga menggunakan My SQL sebagai database server. Awal pembuatan sistem informasi manajemen adalah menyusun database dan merancang tampilan web. Kemudian disusun dan di design menjadi sistem infomasi managemen yang siap digunakan. Dari hasil pembuatan dan pengujian web aplikasi ini didapatkan bahwa data yang sudah diinputkan oleh pengguna dalam pelaporan, dan dapat menunjukkan data rekap laporan pendataan serta kapan pegawai dapat kembali mendapatkan pergantian APD yang baru dengan menggunakan perhitungan MTTF. Disamping itu form laporan dapat dicetak yang kemudian dapat dievaluasi di PT. Pelindo III Cabang Tanjung Perak oleh staf SMI. Kata kunci: alat pelindung diri (APD), database server MySQL, mean time to failure (MTTF), PHP, sistem informasi manajemen ABSTRACK PT Pelindo III Branch Tanjung Perak is the company within the scope of STATE-OWNED ENTERPRISES in indonesia. This company is the second largest port in Indonesia and became the center of the production and distribution of domestic and international goods. Due to being the second largest center of production and distribution and has a high potential hazard, PT Pelindo III Tanjung Perak branch has committed in the implementation of K3 (occupational safety and health) on the work place region. The existence of evidence of the implementation of K3 is with the procurement of personal protective equipment (PPE). Management information system of protective tools themselves can be created with a web-based media. Information is created through the web that all workers can figure out information self protective tools used have entered the exchange time limit based on the calculation of mean time to failure (MTTF) of data exchange tools protective self employees. The creation of management information systems created using the PHP Hypertext Preprocessor as programming language and also use My SQL as the database server. The beginning of the creating of the management information system is a compiled database and designed web interface. Then compiled and designed to become a management information system which is ready to use. The results from the creating and testing this web applications, it obtained that data already inputed by the user in the reporting, and it can show recap report on collecting along with when an employee may return to the turn of the new APD using the calculation of MTTF. In addition, the form report can be printed then be evaluated in PT Pelindo III Branch Tanjung Perak by SMI staff. Keywords: Personal Protective Equipment (PPE), PHP, Management Information Systems.
