ABSTRAK PLTP. Geo Dipa Energi Unit Patuha merupakan perusahaan dalam lingkup BUMN. Perusahaan Pembangkit Listrik Tenaga Panas Bumi (PLTP) ini menghasilkan energi listrik sebesar 55 MW tiap bulannya untuk di jualkan ke perusahaan negara yaitu PLN. Dikarenakan menghasilkan produk energi listrik dan memiliki potensi bahaya tinggi maka diperlukan upaya untuk meningkatkan kedisplinan staff HSE dalam mendokumentasikan dokumen surat ijin kerja aman yang terdiri dari formulir JSA dan Work permit harus tersimpan dengan rapi, aman serta efisien dan efektif. Untuk itu, perlu dilakukan penyediaan Sistem Informasi Manajemen JSA dan Work permit dengan sitem penyimpanan database agar aman dan mudah di akses. Untuk mendapatkan Sistem Informasi Manajemen JSA dan Work permit dengan sistem penyimpanan database agar aman dan mudah di akses maka akan di buat sistem formulir surat ijin kerja aman online yang menyediakan formulir Job Safety Analysis, formulir Work permit, serta view permit list dan user profile list yang tersedia di halaman utama admin menggunakan PHP dan MySQL. Dari hasil pengujian Sistem Informasi Manajemen JSA dan Work permit ini, ketika semua user telah mengakses sistem tersebut maka dibuatlah kuesioner yang bertujuan untuk mengetahui perbedaan dari sistem pembuatan Surat Ijin Kerja Aman (SIKA) yang lama dengan sistem yang baru yaitu Sistem Informasi Manajemen JSA dan Work Permit. Kemudian hasil dari kuesioner oleh 25 responden yang telah mengakses SIM JSA dan Work Permit menunjukan bahwa lebih besar jumlah kenyataan daripada jumlah harapan, Sehingga dapat disimpulkan bahwa SIM JSA dan work permit bermanfaat untuk diterapkan di perusahaan. Geothermal Electricity Power Plant (PLTP) generates 55 MW electrical energy for every month and it is sold to the National Electricity Company (PLN). Because it generates electrical energy and it has a high potential danger, so it needs to improve the discipline of HSE staffs in documenting the secure work permit documents which consist of JSA forms and Work permit should be stored neatly, safely, efficiently and effectively. Therefore, it is necessary to provide JSA Information Management System and Work Permit with the database storage system so that it will be stored safely and accessible. In order to get the secured and accessible database storage system, it woul d be created a secured online work permit form which provides Job Safety Analysis forms, Work permit forms, view permit list and user profile list which are available on the admin homepage using PHP dan MySQL. From the test results of the JSA Information Management System and Work Permit, when all of the users accessed the system, the questionnaire is made which aims to find out the differences between the old secure work permit-making system with the new one, that is, the JSA information management system and work permit. Then, the result of the questionnaires from 25 employees who accessed SIM shows the higher reality rather than hopes. So, it can be concluded that SIM JSA and work permit are beneficial enough to be applied in the company. Keywords : PHP, MySQL, Job Safety Analysis, Work Permit
