Abstract

ABSTRAK Perusahaan ini bergerak dibidang penyedia jasa operator alat berat seperti Head Truck, Reach Stacker, dan Side Loader. Saat ini Perusahaan mengendalikan 37 Unit Head Truck dan 5 Unit Alat Angkat untuk kegiatan bongkar muat kapal di pelabuhan. Berdasarkan dari sumber data kasus kecelakaan Head Truck yang terjadi pada Perusahan ini mulai bulan Januari 2016 sampai dengan November2016, terdapat 30 kasus kecelakaan Head Truck dengan berbagai kasus kecelakaan. Kasus tabrakan head truckmerupakan kasus kecelakaan yang paling sering terjadi.Pada tugas akhir ini dilakukan analisa kecelakaan tabrakan unithead truck pada bulan Januari 2016 sampai dengan bulan November 2016. Untuk mengetahui faktor penyebab permasalahan dari 7 kasus kecelakaan dari segi teknis ataupun manajemen pada tabrakan head truck, digunakan metode Event and Causal Factor Analysis (ECFA). Metode selanjutnya untuk mengetahui tingkat level manajemen maka menggunakan metode Tier Analysis. Hasil analisa yang telah dilakukan terhadap 7 kasus tabrakan head truck menggunakan metode Event and Causal Factor Analysis (ECFA) rata-rata penyebab dasar kasus kecelakaan dikarenakan kurangnya maintenance dan perencanaan sekitar container yard. Selanjutnya rata-rata hasil analisa menggunakan metode Tier Analysis yaitu kurangnya pegawasan terhadap pekerja operator. Perlu adanya sanksi dan reward untuk memotivasi operator bekerja dengan baik. ABSTRACT This company is enrolled in service providers of heavy equipment such as Head Truck, Reach Stacker, and Side Loader. Currently this company controls 37 Units of Head Truck and 5 Units of Lifting Equipment for ship loading and unloading activities at the port. Based on the data source of Head Truck accident on January 2016 until November 2016, there were 30 cases of Head Truck accidents with various types of accident cases. The head truck crash case is the most common accident case compared to other cases.In this final project, the writeranalyzed accident crash of head truck unit on January 2016 until November 2016. In order to know the causal factor of 7 cases of accidents from technical aspect or management at head truck crash, the writer used Event and Causal Factor Analysis (ECFA) method. After found the causal factors of the problems then continued by analyzing with Tier analysis method to determine the management level which responsible for each accident case.The analysis results of 7 cases for head truck crash using Event and Causal Factor Analysis (ECFA) method that the average base cause of accidents cases due to lack of maintenance and planning around the container yard. Furthermore, the analysis average result using Tier Analysis is lack of supervision to the operator workers. It needs reward and punishment to motivate operators so they will work properly.
