Abstrak

Amoniak adalah salah satu bahan kimia yang masuk ke dalam kategori B3 (Bahan bahaya dan beracun). PT Petrokimia adalah salah satu perusahaan yang menggunakan Amoniak untuk bahan baku produksinya, dan melakukan import amoniak. Bahya amoniak adalahmenyebabkan kulit melepuh, iritasi pada mata, kulit dan saluran pernafasan, serta ledakan (explosion) apabila sejumlah ammonia bebas di udara. Dari uraian bahaya amoniak tersebut membuktikan pada proses bongkar muat yang ada di pelabuhan PT Petrokimia Gresik memiliki memiliki resiko bahaya yang cukup tinggi yang dapat membahayakan pekerja dan lingkungan sekitar Untuk mengidentifikasi dan menganalisa risiko ? risiko tersebut dalam penelitian ini menggunakan analisis FMEA (Failure Mode


Abstract

Ammonia is one of the chemicals material which in B3 category (bahan ,berbahayadanberacun) Petrokimia Holding company using ammonia for their productivity material and import it. Ammonia caused skins blister, eyes irritation, skin irritation, respiratory disorders and explosions if some free ammonia in the air reach 16-17 % based on that fact, unloading process gas in Petrokimia Holding company have a high risk level for their worker and their environment for identify and analyze the risks - the risk in this study using the analysis FMEA (Failure Mode and Effect Analysis) and then do the assassment risk, analyzed again with mengguanakan technique FTA (Fault Tree Analysis) and ETA (Event Tree Analysis). By developing a series of accidents result from the Event Tree Analysis (ETA) and develop the basic cause of Fault Tree Analysis (FTA), it can be done Cause Consequence analysis (CCA), so that the sequence of events can be evaluated cut his set. The result identification hazard with FMEA is MLA explosion. Pipe line ammonia leak, and pump leak. And the result from CCA with initianing event MLA explosion is 12 type consequence and 27 cut set and then pipe line ammonia is 12 type consequence and 20 cut set and pump leak is 6 type consequence and 12 cut set. Finally we do structural and non structural mitigation from CCA.Mitigation structural more focused on the recommendations in the form of technology ( any safety equipment and its equipment ) .Care a tool ( maintenance equipment ) as well as other recommendations that directly touch with the object or by a tool ( direct to object ) while in mitigation non structural includes recommendations relating to a system the procedure Key Word:CCA,FaultTreeAnalysis,Event TreeAnalys, Failure Mode and Effec Analysist
