Abstrak

PT Petrokimia Gresik memiliki beberapa distribusi produk yang dihasilkan kepada distrisbutor maupun konsumen. Salah satu distribusi produk yang dilakukan adalah distribusi bahan kimia cair yakni ammonia melalui truk tangki atau container. Bahaya ammonia seperti menyebabkan kulit melepuh, iritasi pada mata, kulit dan saluran pernafasan, serta ledakan (explosion) apabila sejumlah ammonia bebas di udara dan mencapai volume gas ammonia 16-17%. Dari uraian bahaya ammonia tersebut membuktikan bahwa proses pengisian dan pengangkutan ammonia melalui truk tangki memiliki banyak risiko bahaya yang dapat menimbulkan kecelakaan kerja bagi pekerja di sektor pengisian (loading) dan pengangkutan Ammonia. Untuk mengidentifikasi dan menganalisa risiko ? risiko tersebut dalam penelitian ini menggunakan analisis FMEA (Failure Mode


Abstract

PT Petrokimia Gresik has some distribution product that produced for distributor and consumers. One of distribution is the distribution of products made of liquid chemicals through the ammonia tank truck or container. Ammonia hazards such as causes blisters, irritation of eyes, skin and respiratory tract, and the explosion when the number of free ammonia in the air and ammonia gas volume reaches 16-17%. From the description of the hazards of ammonia to prove that the process of charging and transport of ammonia by tank truck has many hazards that can cause accidents at work for workers in loading sector and the transport of ammonia. To identify and analyze the risks in this study using FMEA (Failure Mode and Effect Analysis) and FTA (Fault Tree Analysis) as a risk assessment, then readiness assessment by CAR (Capability Assessment for Readiness) to show the readiness of company with disaster. Both of that hazard identification as a reference to determine the structural mitigation measures and non-structural mitigation. From the results of the FMEA risk identification can be determined using the highest level of risk is fail close of Flow Control Valve and the biggest impact is the tank exploded. And the results of risk identification using FTA as a continuation of the FMEA been a combination of minimal cut sets from the Failure of the process of loading ammonia FTA and tank explodes FTA for structural mitigation to be implemented. While the results of the readiness assessment found that the assessment of the level of emergency preparedness that is fully capable of 47%, 53% very capable, 0% Generally Capable, % Marginally Capable , 0% Not Capable, and 0% Not Applicable. From the results of the readiness assessment is used as a reference in determining the non-structural mitigation. Keyword : loading ammonia, mitigation, risk assessment, readiness assessment
