ABSTRAK PT. SASMITO merupakan perusahaan jasa konstruksi, yang berusaha untuk menjaga keamanan, keselamatan dan kenyamanan para pekerja ditempat kerja yang memiliki potensi bahaya. Namun pada lapangan proyek sering terjadi kecelakaan kerja yang disebabkan oleh human error seperti bekerja dalam keadaan terburu-buru, sikap kerja yang salah dan tidak menggunakan APD (Alat Pelindung Diri). Penelitian ini bertujuan untuk menganalisa human error yang mengakibatkan terjadinya kecelakaan kerja pada pekerja kuli saat melakukan pembongkaran atap. Metode yang digunakan dalam penelitian ini yaitu metode HEART yang digunakan untuk mengetahui probabilitas kegagalan pekerja kuli dalam melaksanakan pekerjaannya. Dari penelitian ini, diketahui prediksi human error yang terjadi saat pekerja kuli memeriksa kayu reng atau usuk dengan probabilitas sebesar 0,322 dan menggunakan safety belt / body harness dengan probabilitas sebesar 0,051. Untuk meningkatkan keandalan pekerja kuli yang rendah dari human error probability yang tinggi pada pekerjaan membongkar atap maka perlu adanya pembuatan SOP (Standard Operational Procedure) dimana fungsi dari SOP ini yaitu untuk membuat keseragaman pola kerja dan keseragaman kualitas dari sebuah proses atau produk yang akan dibuat untuk dilaksanakan. SASMITO is a construction services company, that tried to maintain security, safety and comfort of employees at work that has the potential danger and could happen to anybody, anywhere and anytime. But at the field project, an accidents often occurred and supposedly caused by human error such as work in a hurry situation, the wrong work attitude and not using self protecting devices. This research aimed to analyze human error which caused working accidents operators in extrusion station. Methods used in this research is HEART method which used to know the probability of failure construction employee in performing the job. In this research known the prediction of human error that could be happened when they checking the woods with probabilities of 0,0322 and using safety belt / body harness with probabilities of 0,051. To improve the reliability of low construction employee from the high human error probability in the work of dismantling the roof it is necessary to create SOP (Standard Operational Procedure) where the function of this SOP is to create uniformity of work patterns and uniformity of quality processes or product to be made for action. .
