ABSTRAK PT. Petro Jordan Abadi adalah perusahaan yang bergerak di bidang industri kimia yang memproduksi bahan kimia seperti asam sulfat, asam pospat, H 2 SiF 6, gypsum. Pada produksi asam pospat terdapat masalah produksi yang disebabkan tingginya unscheduled downtime. Karena itu diperlukan perbaikan untuk mengatasi permasalahan downtime tersebut. Line availability digunakan untuk mengukur ketersediaan waktu proses produksi pada phosphoric acid plant secara menyeluruh. Setelah diketahui persentase nilai line availability rate, selanjutnya mencari unscheduled downtime yang paling berpengaruh terhadap nilai line availability rate dengan menggunakan diagram pareto. Hasil diagram pareto yang berupa critical downtime akan dianalisis menggunakan metode Failure Mode & Effects Analysis (FMEA) untuk mengetahui mode kegagalan dan penyebab kegagalan dari sebuah peralatan. Untuk mencegah terjadinya unscheduled downtime maka dibuat usulan rekomendasi perbaikan berdasarkan konsep Total Productive Maintenance (TPM) pada phosphoric acid plant. Hasil perhitungan nilai line availability rate pada tahun 2015 adalah sebesar 67,33% dan masih dibawah standar JIPM. Critical downtime diperoleh 11 penyebab dan 9 diantaranya dikarenakan oleh kegagalan peralatan. Mode kegagalan yang terjadi dari 9 peralatan tersebut yaitu dikarenakan kebocoran, tersumbat, korosi, terkikis, aus, retak, patah, hubung singkat. Rekomendasi perbaikan yang diusulkan untuk menanggulangi mode kegagalan yang terjadi yaitu dengan menggunakan konsep TPM dengan melakukan pemeliharaan mandiri oleh operator, menambah jadwal untuk planned maintenance, kualitas bahan baku untuk produksi, mengurangi pembebanan mesin, menambahkan sensor untuk mengetahui penyebab kegagalan secara dini, memberikan pelatihan untuk meningkatkan kemampuan operator. Petro Jordan Abadi is company that runs in the area chemical industry that produce as sulphuric acid, phosphoric acid, H 2SiF 6, and gypsum. The productions phosphoric acid have a problems productions caused by high unscheduled downtime. Therefore necessary repairs to solve the downtime problems. Line availability used to measure the availability time process of production at phosphoric acid plant overall. After that the know percentage value line availability rate, next find unscheduled downtime has the most influence on line availability rate by using pareto diagram. Pareto diagram results of critical downtime will be analyzed using Failure Mode & Effects Analysis methods to know failure mode and failure causes from an equipment. To prevent the occurrence of unscheduled downtime then made an improvement recommendations based Total Productive Maintenance concept at phosphoric acid plant. The results calculation value of line availability rate in 2015 is 67,33% and still less than JIPM standart. Critical downtime obtained 11causes and 9 of them caused by equipment failure. Failure mode that have occurred from 9 equipment is due to leak, blocking, corrosion, eroded, worn/ napless, crack, broken, short circuit. The improvement recommendations for resolve failure mode that happened is using TPM concept with doing autonomous maintenance by operators, increase the scheduled for planned maintenance, the quality from raw materials for production, reduce imposition machine, adding a sensor to determine failure causes in premature manner, give training to improve the ability operator.
