Abstract

ABSTRAK PT. X merupakan salah satu perusahaan terbesar di Indonesia yang bergerak di bidang pelayaran. PT. X mengoperasikan 66 kapal berbagai jenis yang telah dilengkapi, dengan sertifikasi Biro Klasifikasi Indonesia (BKI), International Safety Management (ISM) Code dan International Safety Port System (ISPS) Code. Berdasarkan sumber data kasus kecelakaan kapal yang terjadi di PT. X pada bulan Januari tahun 2015 hingga bulan Agustus tahun 2016, tabrakan kapal merupakan kasus kecelakan yang paling sering terjadi dibandingkan dengan yang lain. Pada tugas akhir ini dilakukan analisa kecelakaan tabrakan kapal kargo kontainer yang terjadi di PT. X pada pada bulan Januari tahun 2015 hingga bulan Agustus tahun 2015. Untuk mengetahui faktor-faktor penyebab permasalahan dari 8 kasus kecelakaan tabrakan kapal, digunakan metode Event and Causal Factor Analysis (ECFA). Setelah ditemukan faktor-faktor penyebab permasalahan dari tiap kasus kecelakaan tersebut, dilakukan analisa terhadap penerapan standar International Safety Management (ISM) Code. Hasil analisa yang telah dilakukan terhadap 8 kasus kecelakaan tabrakan kapal menggunakan metode Event and Causal Factor Analysis (ECFA) dapat diketahui bahwa terdapat ketidaksesuaian terhadap penerapan standar International Safety Management (ISM) Code pada klausul 5.1; 5.2; 6.1; 6.2; 6.4; 6.7; 7; 8.1; 10.3. Perusahaan perlu meningkatkan implementasi standar International Safety Management (ISM) Code agar terciptanya kondisi keselamatan di kapal dan mencegah terjadinya kecelakaan terulang kembali. Kata kunci: Event and Causal Factor Analysis (ECFA), International Safety Management (ISM) Code, Tabrakan. ABSTRACT PT. X is one of the largest companies in Indonesia which is engaged in shipping. PT. X operate 66 ships of various types that have been equipped with the certification from Biro Klasifikasi Indonesia (BKI), International Safety Management (ISM) Code and International Safety Port System (ISPS) Code. Based on the data source ship accidents that occur in the PT. X in January 2015 until August 2016, ship collision accident is mostly happen than other case. In this thesis, author is going to analysis container cargo ship collision accident that occurred in PT. X in January 2015 to August 2015. To determine the factors that cause the problems of the 8 cases ship collision accident, author used method of Event and Causal Factor Analysis (ECFA). Once found the factors that cause the problems of each case the accident, conducted an analysis of the regulatory International Safety Management (ISM) Code. The result of the analysis that has been done to the ship collision accident 8 cases using the method of Event and Causal Factor Analysis (ECFA) it can be seen that there is a mismatch of the application of standarts of the International Safety Management (ISM) Code in clause 5.1; 5.2; 6.1; 6.2; 6.4; 6.7; 7; 8.1; 10.3. Companies need to improve the implementation of International Safety Management (ISM) Code for the creation of safety conditions on the ship and prevent accidents happening again.
