ABSTRAK PT.Kuda Inti Samudrera Perusahaan yang bergerak di bidang jasa kepelabuhan yaitu pekerjaan maintenance head truck dan alat angkat, serta operasional head truck kontainer . Keselamatan dan Kesehatan Kerja (K3) sangat di perhatikan oleh perusahaan ini. Banyak pekerjaan yang dilakukan di perusahaan ini memiliki Not Acceptable Risk salah satunya adalah kegiatan operasional. Hasil Analisa resiko menunjukkan kegiatan tersebut memiliki resiko Not Acceptable Risk karena sangat sering terjadi kecelakaan truck kontainer yang disebabkan oleh banyak faktor. Dari pihak manajemen K3 terus mengupayakan agar tingkat kecelakaan dapat diturunkan, yaitu dengan mengoptimalkan safety patrol, pre-operation, worker condition, dll. Namun terdapat permasalahan yaitu belum tersedianya pelaporan near-miss serta pelaksanaan pelaporan pre-operation yang belum efektif dan efisien oleh karena itu diperlukan sistem informasi untuk mempermudah pelaporan. Pada Tugas Akhir ini, Perancangan Sistem Pelaporan Pre-operation dan Near-miss berbasis android dan web sebagai solusi permasalahan tersebut sehingga pelaporan lebih efektif dan efisien. Perancangan aplikasi dengan mengunakan pendekatan metode SDLC (System Development Life Cycle) dalam penelitian ini seperti requirement, design, implementation, testing.Pembuatan aplikasi android dengan android studio dan bahasa pemrograman java, perancangan database mengunakan MySQL, pembuatan web dengan bahasa pemrogramanya PHP. User untuk Android ini adalah operator dan User web adalah supervisi operator, supervisi mekanik, dan safety officer. Android sebagai media pelaporan dan Web sebagai media untuk melihat hasil pelaporan. Dengan Aplikasi android, operator lebih mudah melaporkan pre-operation dan near-miss dan dengan web supervisi serta safety officer mudah memantau dan memeriksa pelaporan pre-operation dan nearmiss. Occupational Health and Safety (K3) is considered by the company. Lots of work to do in this company Not Acceptable Risk one of which is the operational activities. The results of the risk analysis showed Not Acceptable Risk , because a very often happens accident container truck caused by many factors . From the management K3 continue to strive for the accident rate can be reduced, that by optimizing safety patrols, pre-operation, worker condition, etc. But there are problems that the unavailability of nearmiss reporting and the reporting of pre-operation has not been effective and efficient therefore it is necessary to simplify the reporting of information systems.In this Final Project, design of Reporting System Pre-operation and Near-miss based on Android and the web as a solution to these problems, so that reporting is more effective and efficient. Application design method approach SDLC (System Development Life Cycle) in this study such as requirements, design, implementation, testing.Making android application with android studio with the Java programming language, database design using MySQL , making the web with bootstrap template with the PHP programming language. Users of Android is the operator and the user web are supervision of operator, supervision of mechanical and safety officer. Android as media reporting and the Web as a medium to see the results reporting. With the android application, operator easier to report pre-operation and near-miss and with web supervision and safety officer easily monitor and check the reporting of pre-operation and nearmiss.
