Abstract

ABSTRAK PT.Liku Telaga merupakan suatu perusahaan asosiasi dari grup PT.Lautan Luas Tbk yang bergerak dalam bidang produsen bahan kimia. Produk yang di hasilkan antara lain asam sulfat, sodium silicat, almunium sulfat. Penelitian ini berfokus pada identifikasi bahaya yang ada di unit Sodium Silicat Production (SSP) di PT.Liku Telaga Gresik. Risiko akan didapatkan dari pengkalian dampak dan peluang. Untuk mengidentifikasi dan menganalisa risiko-risiko tersebut dalam penelitian ini menggunakan analisis FMEA (Failure Mode & Effect Analysis) dan FTA (Fault Tree Analysis). Kemudian dilakuakan penilaian tingkat kesiapsiagaan bencana atau readiness assessment menggunakan metode CAR (Capability Assesment for Readiness). Setelah diketahui penyebab dasar dari kegagalan tersebut dilanjutkan dengan pemberian rekomendasi berupa mitigasi struktural dan mitigasi non struktural. Dari hasil identifikasi risiko (Risk Assessment) menggunakan FMEA dapat diketahui bahwa nilai risiko tinggi yaitu terdapat pada kegagalan pompa produksi dengan nilai risiko 20, kebocoran pada pipa transfer dengan nilai risiko 16, kebocoran tangki disolver dengan nilai risiko 15. Termasuk kategori risiko tinggi karena nilai risiko >12 – 25. Identifikasi menggunakan FTA dari kegagalan pompa produksi didapatkan 7 cut set dan 5 minimal cut set, pada kebocoran line pipa transfer didapatkan 13 cut set dan 9 minimal cut set. kebocoran tangki dissolver didapatkan 12 cut set dan 2 minimal cut set. Berdasarkan hasil Readiness Assessment menggunakan CAR Checklist diketahui bahwa penilaian tingkat kesiapsiagaan keadaan darurat yaitu 22 pernyataan pada kategori very capable dan 3 pernyataan Marginally Capable. Mitigasi struktural berupa rekomendasi pada penipisan material tangki, motor pada pompa terbakar, ball valve gagal membuka. Sedangkan mitigasi non struktural meliputi rekomendasi yang berhubungan dengan sistem manajemen perusahaan yaitu pemberian pelatihan rutin pada pekerja, identifikasi risiko dilakukan secara terjadwal, penyediaan peralatan keselamatan, jadwal inspeksi rutin, pemberian pelatihan pada masyarakat sekitar. Mitigasi non struktural diusulkan berdasarkan hasil readiness assessment. Kata kunci: CAR, FMEA, FTA, mitigasi non-struktural, mitigasi struktural. ABSTRACT PT. Liku Telaga is an association company of PT. Lautan Luas Tbk group which produce chemical material. The product which is made by PT. Liku Telaga are sulfuric acid, sodium silicate, and sulfuric alloy. This research focused on hazard identification which available at Sodium Silicate Production (SSP) of PT. Liku Telaga. The risks will be known by multiplication of probability and severity. To identify and analyze the risks, researcher used FMEA (Failure Mode & Effect Analysis) and FTA (Fault Tree Analysis). After that, do readiness assessment with CAR (Capability Assessment for Readiness). Then recommendation must be given as structural mitigation and non structural mitigation. From the result of Risk Assessment using FMEA, researcher know the the highest risk score happen at production pump failure with the risk score of 20, leak from transfer pipe with the score of 16, and disolver tank leak with the score of 15. The risk categorized as high risk because te score >12-25. From the identification using FTA with the major event production pump failure, there are 7 cut sets and 5 minimal cut sets. 13 cut sets and 9 minimal cut sets from pipe line transfer failure. 12 cut sets and 2 minimal cut sets from disolver tank failure. Based on Readiness Asessment with CAR Checklist, researcher identify the score of emergency response score were 22 very capable category and 3 marginally capable catagory. Structural mitigation are tank material dilution, burned motor pump, and ball valve fail to open. Non Structural Mitigation are recommendation which have relation with company management system like workers routine training, scheduled risk identification, PPE management, scheduled routine inspection, society training.
