Abstract

ABSTRAK PT. X adalah perusahaan yang bergerak dibidang industri baja didirikan di Sidoarjo Indonesia tahun 1976, produk PT. X hasil fasilitas modern pembuatan baja melalui tanur listrik yang bebas slag, metallurgi sekunder dan pengecoran continue untuk baja yang bersih. Dimana setiap proses produksi di PT. X diselimuti oleh standart Incident Investigation bersumber dari Fatality Prevention Audit (FPA) acuan standart audit PT. X. Berdasarkan sumber data kasus kecelakaan yang terjadi di PT. X pada bulan Januari 2014 hingga bulan Juni 2016, hilangnya waktu kerja akibat kecelakaan merupakan kasus yang paling sering terjadi. Pada tugas akhir ini dilakukan analisa pada kecelakaan menghilangkan waktu kerja yang terjadi di PT. X pada bulan Januari 2014 hingga bulan Juni 2016. Untuk mengetahui faktor-faktor penyebab permasalahan dari 11 kasus kecelakaan menghilangkan waktu kerja, digunakan metode Event and Causal Factor Analysis (ECFA). Hasil analisa metode (ECFA) berupa Dirrect Cause, Root Cause, Contributing Cause, dihubungkan pada level dan poin yang terdapat pada Incident Investigation bersumber dari Fatality Prevention Audit (FPA) acuan standart audit PT.X. Dari hasil analisa yang telah dilakukan terhadap 11 kasus kecelakaan menghilangkan waktu kerja, diketahui akar permasalahan kasus kecelakaan dikarenakan seperti sebelum awal pekerjaan tidak melakukan identifikasi resiko yang mungkin berasal dari lingkungan, peralatan, dan bahan. Berdasarkan hasil analisa tersebut dapat diketahui bahwa timbul ketidaksesuaian terhadap standart Managing Contractor “level 1, poin 10”, “level 2, poin 1”, dan Hazard Identifications and Risk Assesment “level 0, poin 1”, “level 1, poin 1, poin 9”, “level 3, poin 10”, bersumber dari Fatality Prevention Audit (FPA). Oleh karena itu perusahaan perlu meningkatkan implementasi standart Fatallyti Prevention Audit (FPA) dan memberikan pelatihan-pelatihan terhadap peraturan terkait. Kata kunci: Kecelakaan, Event and Causal Factor Analysis (ECFA), Incident Investigation (FPA). ABSTRACT PT. X is a company moving in the steel industry which established in Sidoarjo, Indonesia since 1976, the product of PT. X come from modern facilities with electric furnace in order to steelmaking complete with slag-free, secondary metallurgy and foundry continue to clean steel. Wherein each of the production process in PT. X is covered by the standard Incident Investigation sourced from Fatality Prevention Audit (FPA) as audit standard reference of PT. X itself. Based on case data sources accident that occurred in PT. X in January 2014 until the month of June 2016, the loss of work time due to accidents are the most common cases. In this final task the accident happened at PT. X with loss working time have been analyzed in range time from January 2014 to June 2016. To determine the factors that cause of problems from the 11 accident cases with loss working time, used methods Event and Causal Factor Analysis (ECFA). The results will appear from the analysis method (ECFA) are in the form of Direct Cause, Root Cause, Contributing Cause, linked to the level and the points contained in the Incident Investigation sourced from Fatality Prevention Audit (FPA) reference to the audit standards of PT.X. From the analysis that has been done to eliminate accidents in 11 cases of working time, known root causes such as accidents due before the beginning of the work does not identify risks that might come from the environment, equipment, and materials. Based on the results of the analysis can be seen that the discrepancy arose against standart Managing Contractor “level 1, poin 10”, “level 2, poin 1”, and Hazard Identifications and Risk Assesment “level 0, poin 1”, “level 1, poin 1, poin 9”, “level 3, poin 10”, sourced from Fataity Prevention Audit (FPA). Therefore, companies need to improve the implementation of standards Fatality Prevention Audit (FPA) and providing training to the relevant regulations.
