ABSTRAK PT. Pelindo III Cabang Tanjung Perak merupakan perusahaan dalam lingkup BUMN di Indonesia. Perusahaan merupakan pelabuhan tersebar ke-2 di Indonesia dan menjadi pusat produksi dan distribusi barang domestik maupun Internasional, dikarenakan menjadi pusat produksi dan distribusi terbesar ke-2 dan memiliki potensi bahaya tinggi yang di sebabkan oleh manusia, lingkungan dan peralatan. Maka diperlukan upaya untuk meningkatkan kedisiplinan staf SMI dalam mendokumentasikan dokumen laporan kecelakaan kerja dengan baik, agar memudahkan staf SMI dalam kesiapan apabila ada audit external maupun internal. Untuk itu perlu dilakukan penyediaan media pelaporan yang sistematis dengan sistem penyimpanan database agar aman dan mudah diakses. Pembuatan web merujuk pada Peraturan Menteri Tenaga kerja 3/MEN/1998 Tentang Tata Cara Pelaporan dan Pemeriksaan Kecelakaan dan Surat Keputusan Direktur Jenderal Pembinaan Hubungan Industrial dan Pengawasan Ketenagaan Kerja Departemen Tenaga Kerja No. Kep. 84/BW1998 Tentang Cara Pengisian Formulir Laporan dan Analisis Statistik Kecelakaan. Untuk mendapatkan media pelaporan yang sistematis dengan sistem penyimpanan database yang aman dan mudah diakses akan dibuat sistem pelaporan online yang menyediakan pelaporan kecelakaan dan safety performance menggunakan MySQL dan PHP. Dari hasil pembuatan dan pengujian web aplikasi ini didapatkan bahwa data yang sudah diinputkan oleh pengguna dalam pelaporan, dapat menunjukkan data rekap laporan kecelakaan kerja dan nilai dari perhitungan safety performance tiap bulan dan tahun. Disamping itu form laporan dapat dicetak dan dijadikan sebagai bahan dalam melakukan analisa pencapaian keselamatan dan kesehatan kerja di PT. Pelindo III Cabang Tanjung Perak oleh staf SMI. Kata kunci: MySQL, PHP, Kecelakaan Kerja, Safety Performance ABSTRACT PT. Pelindo III Tanjung Perak is one of state owned company which focuses in logistic activity. PT. Pelindo III is the 2 nd largest port in Indonesia and it has become the center of production and distribution of domestic and International commodity. As it has become the center of both domestic and international commodity distribution, the potential of high hazard created by human, environtment, and equipment is getting rise. Eventually, Efforts are needed to raise SMI staff discipline in order to report work accident document in a good way as well as ease SMI staff to prepare both external and internal audit well. Therefore, there should be a systematic media report with database storage system in order to be secure and easy to access. Web making refers to Regulation Minister of Labor 3 / MEN / 1998 about Procedure on how to make the Report and Accident Investigation and Decree of General Director of Industrial Relations and Supervision of the Department of Labour Workforce No. Kep. 84 / BW1998 about Method on Filling Report Form and Statistical Analysis of Accident. In order to get a systematic media report with a safe database storage system and easy to access, Online Reporting System would be created which provided accident report and safety performance using MySQL and PHP. After it has been made and tested, this web development resulted that data that has been input by users could show recapitulation of work accident report data and the value of safety performance calculation monthly and yearly. Besides, report form could be printed and used as the object in doing health and safety environment accomplishment analysis toward SMI staff in PT. Pelindo III which operated in Tanjung Perak.
