ABSTRAK PT. Swadaya Graha merupakan Perusahaan Jasa yang didirikan oleh PT. Semen Gresik (Persero), Tbk. Pada saat ini, kurang sistematisnya safety report dan kurang siapnya manajemen ketika dilakukan audit internal maupun eksternal merupakan suatu permasalahan mendasar. Selain itu, safety officer juga harus memperhatikan sistem inspeksi yang dilakukan di Divisi Fabrikasi Baja PT. Swadaya Graha dimana inspeksi yang dilakukan tidak menggunakan form checklist serta tidak adanya wadah modul Inhouse Training untuk mempermudah para direksi ataupun pekerja lainnya memahami pelatihan-pelatihan. Hal tersebut dapat dilakukan secara online melalui smartphone. Web Responsive ini memiliki tujuan untuk menyediakan sarana pelaporan data statistik K3 secara online serta sarana penyimpanan data ke dalam database. Tujuan lainnya menyediakan form checklist inspeksi yang tedapat pada database serta modul Inhouse Training terdapat di dalamnya. Pembuatan web responsive dan sistem dilakukan dengan menggunakan source code bahasa pemrograman PHP-MySql dan database dibuat dengan program PHP MyAdmin dalam program Xampp. Dari hasil pengujian web aplikasi ini didapatkan bahwa data yang sudah diinputkan oleh pengguna dalam pelaporan, dapat menunjukkan data rekap laporan dan nilai dari perhitungan safety statistik tiap bulan. Disamping itu form laporan, form checklist, dan lembar Job Safety Analysis dapat dicetak dalam format pdf serta tabel inhouse training yang dapat diunduh dari database web aplikasi. Kemudian dapat dianalisa terhadap pencapaian keselamatan dan kesehatan kerja dalam tiap bulan di Divisi Fabrikasi Baja PT. Swadaya Graha oleh pihak safety officer. At this time, less systematic and less unprepared safety report management when internal and external audit is a fundamental problem. In addition, the safety officer must also consider the system of inspections carried out in Steel Fabrication Division PT. Swadaya Graha which do not use the inspection checklist form and the lack of Inhouse Training module to make it easier for directors or other employees understand the training. This can be done online via smartphone. Responsive Web has the goal of providing a means of reporting statistical data K3 online as well as a means of storing data into the database. Other destinations provide inspection checklist form which is attached to the database as well as Inhouse Training modules contained therein. Responsive web creation and the system is done by using the source code programming language PHP and MySql database created with PHP program MyAdmin in Xampp program. From the test results it was found that the web application data that has been entered by the user in reporting, may indicate the data recap report and the value of safety calculation of monthly statistics. Besides, the report form, form checklist, and Job Safety Analysis sheets can be printed in pdf format and inhouse training tables can be downloaded from the web database application. Then can be analyzed to the achievement of health and safety in each month in Steel Fabrication Division PT. Swadaya Graha by the safety officer.
