ABSTRAK Berdasarkan rekapan data kecelakaan kerja PT. X tahun 2016 diketahui bahwa kecelakaan kerja pada proses pekerjaan Manual OAW Cutting adalah jenis kecelakaan yang dapat menimbulkan kerugian yang sangat besar yang disebabkan oleh human error. Untuk mencegah timbulnya kecelakaan kerja diperlukan analisa human error dalam tiap tahapan tugas pekerjaan Manual OAW Cutting. Penelitian ini menggunakan salah satu metode dari Human Reliability Analysis (HRA) yaitu Cognitive Reliability and Error Analysis Method (CREAM ) untuk mendapatkan nilai Cognitive Failure Probability (CFP). Langkah yang dilakukan ada dua cara adalah yaitu basic method dan extended method. Pada tahapan basic method didapatkan nilai CFP secara umum yang didapat dari penilaian Commond Performance Condition oleh expert judgement . Pada tahapan extended method akan didapatkan nilai CFP untuk setiap task. Berdasarkan hasil penelitian menunjukkan faktor-faktor yang mempengaruhi timbulnya error pada pekerjaan Manual OAW Cutting kecukupan organisasi, Time of Day, kecukupan pelatihan dan pengalaman. Aspek kognitif yang memiliki nilai probabilitas error paling tinggi pada pekerjaan Manual OAW Cutting adalah aspek kognitif verify dengan error mode perencanaan yang tidak cukup dengan nilai CFP sebesar 0.0625. Sebagai rekomendasi work instruction sebaiknya dibuat lebih rinci, melakukan safety patrol secara rutin pada saat mengoperasikan peralatan pengelasan, wajib memakai APD sesuai dengan potensi dan resikonya, memberlakukan shift kerja dan mengatur waktu istirahat yang cukup, melakukan pelatihan penyegaran seperti seminar dan sosialisasi. X in 2016, is it shown that the workplace accident happen during a manual process of Manual OAW is an accident that could cause a significant loss that triggered by Errors. Human error analysis in each stages of the Manual OAW Cutting is done in order to avoid the workplace accident. This research is using method from Human Reliability Analysis (HRA) which is; Cognitive Reliability and Error Analysis Method (CREAM). To get Cognitive Failure Probability (CFP) value, the steps that have to be done is two ways that are basic method and extended method. On the basic method, researcher earn CFP value generally that could be found from Commond Performance Condition evaluation by expert judgement. On extended method stages will be earned CFP value for each task. Based on the research result, it is shown factors that play a part on manual OAW cutting errors. Those are organization sufficiency, time of the day, enough training and experience. The cognitive aspect that has highest error probability on Manual OAW Cutting is verify cognitive aspect with error mode planning that is not enough with 0.00625 CFP value. As a work instruction recommendation, Conduct routine safety patrol when operating welding equipment, must wear PPE in accordance with the potential and risk, apply shift work and arrange enough rest time, conducting refresher training such as seminar and socialization..
