ABSTRAK Chlorination plant merupakan unit penghasil sodium hypochlorite (NaOCl) yang digunakan sebagai pengusir biota laut yang akan masuk pada inlet CWP (circulating water pump) pada PT. PJB UP Gresik. Terjadinya gangguan pada komponen mesin dalam system chlorination plant akan berpotensi menghambat produksi sodium hypochlorite (NaOCl), sehingga banyak kerugian yang didapat perusahaan jika hal tersebut terjadi. Oleh sebab itu perlu dilakukan kegiatan perawatan yang tepat dan terencana agar chlorination plant dapat berfungsi secara opimal. Metode penelitian yang digunakan adalah Reliability Centered Maintenance II dengan memadukan analisis kualitatif yang meliputi FMEA dan RCM II Decision Worksheet. Serta perhitungan kuantitatif diberikan untuk menentukan perawatan optimal (TM) dengan memperhatikan biaya perawatan (CM), biaya perbaikan (CR), MTTR serta parameter distribusi setiap komponen. Sedangkan HIRARC dan JSA digunakan untuk mengidentifikasi bahaya dan membantu dalam menganalisa potensi bahaya kerja pada saat melakukan kegiatan perawatan pada chlorination plant. Berdasarkan hasil penelitian diketahui terdapat 25 failure mode yang dapat menyebabkan function failure pada chlorination plant. Pada penentuan kegiatan perawatan pada RCM II didapatkan bahawa terdapat 17 failure mode dapat dicegah dengan scheduled discard task, 7 failure mode dapat dicegah dengan scheduled restoration task, dan 1 failure mode dapat dicegah dengan schedule condition task. Analisa resiko berdasarkan HIRARC menunjukan bahwa sebagian besar kegiatan perawatan memiliki resiko rendah, hanya ada satu yang memiliki resiko menengah yaitu penggantian packing karet pada sambungan line discharge degas tank, yang selanjutnya pada komponen tersebut dilakukan pembuatan JSA. Kata kunci : Chlorination Plant, FMEA, HIRARC, JSA, RCM II Decision Worksheet ABSTRACT Chlorination plant is a unit which production sodium hypochlorite (NaOCl) used as a marine biota repellent that will enter the CWP inlet (circulating water pump) at PT. PJB UP Gresik. The occurrence of interference in machine components in the chlorination plant system will potentially inhibit the production of sodium hypochlorite (NaOCl), resulting in many losses to the company if it happens. Therefore, it is necessary to carry out proper and planned maintenance activities so that chlorination plant can function optimally. The research method used is Reliability Centered Maintenance II. This method combine between qualitative analysis that includes FMEA and RCM II Decision Worksheet. And quantitative calculations are given to determine optimal maintenance (TM) with respect to maintenance costs (CM), repair costs (CR), MTTR and distribution parameters of each component. While HIRARC and JSA are used to identify hazards and assist in analyzing potential occupational hazards during the maintenance of the chlorination plant. Based on the result of the research, there are 25 failure modes which can cause the failure function at chlorination plant. In the determination of the maintenance activity in RCM II it was found that 17 failure modes can be prevented with scheduled discard task, 7 failure modes can be prevented with scheduled restoration task, and 1 failure mode can be prevented by the schedule condition task. Risk analysis based on HIRARC shows that most maintenance activities have low risk, there is only one that has medium risk that is replacing rubber packing in connection line discharge degas tank, then on that component made JSA.
