ABSTRAK Industri home and personal care industry Surabaya merupakan perusahaan yang memproduksi sabun dengan bahan baku utama caustic soda. Proses distribusi caustic soda mengalami perubahan pada sistem sehingga diperlukan adanya identifikasi bahaya ulang untuk mengetahui apakah terdapat perubahan risiko pada proses tersebut.Penelitian ini menggunakan metode Hazard and Operability (HAZOP) untuk mengidentifikasi potensi bahaya yang mungkin terjadi dan level dari bahaya tersebut yang kemudian dilanjutkan dengan metode Layer Protection Analysis (LOPA) untuk mengevaluasi kecukupan lapisan perlindungan yang sudah ada. Hasil HAZOP dengan resiko paling tinggi dilanjutkan dengan penentuan nilai SIL (Safety Integrity Level) yang didapatkan dari nilai Probability Failure on Demand (PFD). Hasil HAZOP menunjukkan 5 consequences yang berada pada level 1 (high risk) yang selanjutnya dijadikan skenario LOPA. Perhitungan frekuensi skenario didapatkan bahwa pada skenario satu pada kategori optional yaitu resiko masih dapat diterima, skenario dua, tiga, empat, dan lima berada pada kategori action at next opportunity yang berarti diperlukan tindakan pengurangan yaitu penambahan protection layer berupa SIF pada skenario dua, tiga, empat dan lima dengan penurunan risiko sebesar 10 -2 . Dari PFD yang telah ditetapkan dapat disimpulkan bahwa skenario satu, dua, tiga, empat dan lima nilai SIL adalah 1. Selain itu inspeksi berkala dan pelatihan dapat dipertimbangkan untuk mencegah terjadi kegagalan yang membahayakan. Penelitian juga menghasilkan sistem perpipaan dengan jenis material stainless steel schedule 40 dengan jenis valve ball valve dan pompa deep packing gland atau double mechanical seal. Kata Kunci: Pabrik Produsen Sabun, Caustic Soda, HAZOP, LOPA, SIF, SIL ABSTRACT Home and personal industriy in Surabaya are the factory that produce soap with caustic soda as raw material. Caustic soda distribution has changed its system so that re-hazard identification is needed to determine whether there is a risk change in the process. This research used Hazard and Operability (HAZOP) method to identify risk potential that might be happened and level of its risk. Following the previous action, this study also evaluated the sufficiency of existing protection layer by using Layer Protection Analysis (LOPA) method. The result of HAZOP with the highest risk then was continuously examined by determining the grade of SIL (Safety Integrity Level) attained from Probability Failure on Demand (PFD) grade. The HAZOP result showed five consequences at level one (high risk) which would be schemed as LOPA scenario. Based on scenario frequency calculation, scenario one was included in ‘optional’ category, which contained very low risk consideration;. The scenario two and three, four, and five were included in ‘the action at next opportunity’ category. Reduction action was required by adding SIF for scenario two three, four and five which could reduce risk until 10 -2 . Based on controlled PFD, for scenario one, two,three, four and five can be concluded that the grade of SIL was 1. Besides, periodic inspection can be considered to prevent fatal and dangerous failure. Furthermore, this study was also aimed to select the type of material piping and pumping system. In conclusion, the suitable types are stainless steel pipe schedule 40, with valve ball valve model; and deep packing gland or double mechanical seal pump.
