ABSTRAK PT XYZ merupakan perusahaan sub contractor yang bergerak di bidang maintenance. Berdasarkan data kecelakaan kerja tahun 2014-2016, kecelakaan tertinggi ialah pada pekerjaan sandblasting dan pengelasan dengan human error sebagai penyebab dominan. Penelitian ini bertujuan untuk menganalisa probabilitas human error terhadap pekerja untuk meminimalisasi terjadinya kecelakaan kerja. Metode yang digunakan adalah Standardized Plant Analysis Risk Human Reliability Assessment (SPAR-H). Hierarchial Task Analysis dibuat berdasarkan work instruction untuk menilai Human Error Probability (HEP) pada tiap tahapan pekerjaan. Penilaian dilakukan oleh expert sesuai kuisioner dan panduan metode SPAR-H. Nilai HEP tertinggi pada pekerjaan sandblasting pada task Memasang pagar pada scaffolding yang paling atas untuk mengaitkan full body hardness dengan nilai 0,7731. Sedangkan nilai HEP tertinggi pada pekerjaan pengelasan pada task Memastikan menggunakan full body hardness dengan nilai sebesar 0,54778. Dilakukan penilaian dampak (impact assessent) untuk mengidentifikasi konsekuensi yang mungkin terjadi. Nilai HEP digunakan untuk menentukan likelihood dan penilaian severity dilakukan oleh expert untuk mengetahui risk rating pada setiap task pekerjaan. Task dengan risk rating medium (M) dan high (H) dianalisa menggunakan metode Non Compliance Analysis (N-CA) yang menghasilkan rekomendasi : manajemen APD, perbaikan prosedur pekerjaan, pengawasan terhadap pekerja, training, pembuatan checklist, pemasangan safety sign dan papan informasi pekerjaan, inspeksi, dan penerapan 5R. ABSTRACT PT XYZ is a subcontractor company which enrolled in maintenance. Based on the work accident data from 2014-2016, the highest accident rate are sandblasting and welding worker with human error as a dominant cause. The aim of this research is to analyze the probability of human error towards workers in order to minimize the occurrence of work accidents. The method which is used in this research is Standardized Plant Analysis Risk Human Reliability Assessment (SPAR-H). Hierarchycal Task Analysis is made based on a work instruction to determine Human Error Probability (HEP) at each job steps. The expert performs assessment according to the questionnaire and guidance of SPAR-H method. The highest HEP for sandblasting work is Installing the rail on the scaffolding top to link the full body hardness task with 0.7731. On the othe hand, the highest HEP for welding work is to make sure of the hardness task use full body with 0.54778. To identify the possible consequences, it is necessary to do an assesment impact. HEP is used to determine likelihood and severity assessment which is performed by the expert, in order to know the risk rating on of each task. Tasks with medium (M) and high (H) risk rating were analyzed by using the Non Compliance Analysis (N-CA) method which was obtained with recommendations : the Management of Personal Protective Equipment, the improvement of job procedure, supervising the worker, training, creating the checklist, installing the safety sign and worker information board, inspection and 5R implementation.
